A&S Outstanding Performance Award for Staff Nomination Form

You are welcome to nominate any full-time staff member in A&S. Any faculty or staff member can nominate any staff member. Self-nominations are welcome as well. 

Please note if you self-nominate, we will send an email to a person you designate requesting they fill out the this online form in support of your nomination. Your nomination supporter must fill it out for your self-nomination to be considered. It is a good idea to make sure they are willing to support your nomination prior to self-nominating. 

If you choose to nominate someone else, the nominee will be sent an email asking them to fill out this online form in support of their own nomination. The self-statement must be filled out for the nomination to be considered.
