Thrive Community Survey
Thrive is a local non-profit developing an enriching and fulfilling full-time living community in the Piedmont Triad, North Carolina, for adults with intellectual and developmental disabilities (I/DD) that can live with support, but independent of 24/7 care. The vision is to build in the next five years.
Thrive will be a safe, independent community that brings together individuals with I/DD in a positive setting. It’s a big dream, but one that is possible.
Your answers will help shape how Thrive develops. As a special thank you, there will be an option to enter a drawing for one of five Visa gift cards worth $50 each at the end of this survey. Sharing your contact information is optional.
