INFORMED CONSENT FOR PARTICIPATION IN THE ASSESSMENT OF PARENTS PROJECT
PURPOSE OF THE STUDY Participating in this research study involves being asked to complete several questionnaires regarding parenting and parents’ well-being within their child’s first two years of life. This study is being done as part of research project at the Erikson Institute. The purpose of this study is to develop a new measure of beliefs regarding parenting abilities. You are being asked to complete questionnaires on-line as you have identified yourself as an over 18 years of age parent of a birth to twenty-four month old infant/toddler. You will be asked to answer questions about your parenting skills, mood, worries, and support from friends and family. Your participation in this research is voluntary. If you agree to participate by clicking the “I accept” button at the end of this form, you will be connected to the surveys that are specific to your child’s age. Upon completion of the surveys, you will have the opportunity to enter a drawing for one of four $25 Target gift cards. Entry into the drawing is completely voluntary and requires that you submit your email address to be notified if you have won. You are not asked for other identifying information (e.g., name, address, etc.) as part of participating in this study.
METHODS & PROCEDURES Completion of the questionnaires will take approximately 30 to 40 minutes. Before you complete the questionnaires, you are asked to read this document in order to learn the purpose of the study, the meaning and process of the informed consent process, confidentiality, and voluntary participation—which means that you may end your participation in the project at any time, without any penalty or loss of benefits. You are then asked to check the box of acceptance indicating your electronic signature of consent or the box indicating your lack of acceptance of participation. You are also able to print a copy of the informed consent document for your records, should you desire. Should you consent to participate by checking the “I accept” button, you will be forwarded to the website with the questionnaire and will be asked questions about yourself, your parenting and your well-being. Should you not consent to participate by checking the “I do not accept” button, you will be exited from the website. After reading this document, you should feel that all of your questions have been satisfactorily answered, and that you are able to make an informed decision to participate or not participate in the study.
VOLUNTARY PARTICIPATION Your participation in this research project is completely voluntary. You may choose not to take part at all. If you decide to participate in this research, you may withdraw your participation at any time. If you decide not to participate in this study or if you stop participating at any time, you will not be penalized. However, only those who complete the survey will be given the option to enter their email address to participate in the drawing.
RISKS or DISCOMFORTS There are few risks to participating in this study. You may find some of the items in the questionnaire difficult or uncomfortable to answer. If any of the questions make you uncomfortable, you may skip them. Please keep in mind that there are no right or wrong answers. The researcher is interested in your experiences. You will also be asked for information about yourself, your parenting partner (if applicable) and your infant/toddler. If completing these questionnaires results in concern regarding your well-being, we encourage you to contact the primary investigator, Dr. Tracy Moran (firstname.lastname@example.org; 312-755 2250, extension 7125) or another professional. You can contact a local physician for a list of referrals for specialists in your area. Your decision regarding whether to seek a referral will not impact your participation in this study.
