Please enter the following information for each applicant. This information helps us to identify and keep track of the applicants from your organization. (This form can be filled out more than once.)
All applicants listed below should have a general recommendation form by you or staff at your organization. Applicants need to add your name and email in the recommender area of the application in order for you to receive a request by email to complete their online recommendation form. Your organization's recommendation should be the second recommendation, in addition to a School Counselor recommendation for high school applicants or the College recommendation for college applicants.
Please encourage your applicants to list your organization on the application when asked "How did you hear about the Phillips Scholarship?"

If you have students completing the New Application (High School or College) please submit this list by 5/15/24.
