In preparation for the Stone Belt Annual Celebration on September 26, 2016, we are seeking nominations for agency-wide award recognitions. We are accepting nominations from all areas of Stone Belt services, and we encourage you to participate in this wonderful opportunity to recognize contributions and achievements from throughout the agency. Please take a few moments to nominate a deserving individual or group. All Stone Belt Arc members, clients and staff are eligible to submit nominations. If you wish to nominate an individual for more than one award, please complete a separate form for each nomination.
Award winners will be selected based on the most compelling case made by the nominator for the specific nomination category. Awards in all categories will be presented at Stone Belt’s agency-wide Annual Celebration.
All information on the form is required; only fully completed nominations will be considered. Deadline for nominations: 11:59 pm on Friday, September 2, 2016.
