The Vaccination Experiences of People with I/DD, DSPs |
Thank you for taking a few minutes to complete this short questionnaire about the state of COVID-19 vaccination among people with intellectual and developmental disabilities (I/DD) and direct support professionals (DSPs) supported by the ANCOR community.
Please note that this survey is intended to be completed by provider organizations. If your organization provides services across multiple states, please complete the questionnaire once for each individual state or site, rather than for all of the people supported by your organization nationwide. If you are a state association professional, we encourage you support this effort by sharing this questionnaire with your members.
The data collected through this questionnaire will help ANCOR advocate for more equitable vaccine allocation and raise awareness about the challenges facing people with I/DD and DSPs in the quest to become vaccinated. Although you will be asked to provide your name and contact information at the end of this questionnaire, all data will be anonymized before sharing, and we will never share your personal information with third parties.
