Every college and university with new undergraduate and graduate students is required to report the immunization status of enrolled students. Please complete the following survey to record the immunization status of all enrolled undergraduate and graduate students in your school. To see the paper form of the survey and instructions, please refer to the Immunization Branch website: http://immunize.nc.gov/schools/air_collegesuniversities.htm.
This survey is 7 pages long and tracks with the Annual Immunization Report. Please fill out that worksheet before starting this survey. The survey should take 15-30 minutes if the worksheet has been completed ahead of time.
You must be connected to a printer if you want to print a copy of the report you are submitting. You cannot go back and access your report once you have submitted it.
