This page allows you access to the BASIC UNFORMATTED VERSION of the AHPEQS. The Commission will soon release a User Guide and supporting documentation to help health services implement the question set.

I have read and agree to use the AHPEQS according to Creative Commons BY-NC-SA International Licence 4.0.

In summary, this means I agree to:
1. Attribute the AHPEQS to the Australian Commission on Safety and Quality in Health Care (the Commission), and to note any modifications made to the AHPEQS without implying endorsement of the changes by the Commission;
2. Only use the AHPEQS or any modifications thereof for non-commercial purposes;
3. Grant the same BY-NC-SA licence, attributing the Commission and noting any changes, for any modified version of the AHPEQS to other users.
