2018 Resident Service and Satisfaction Survey

Dear Resident,

Thank you for participating in the 2018 Resident Service and Satisfaction Survey!

Questions should be answered by the Head of Household. Your participation in this survey is confidential and your individual answers will not be shared with property management, government agencies, or any other groups outside of AHDC.

All households who submit a survey by June 18, 2018 will receive a $5 Giant gift card. The deadline to complete the survey is July 4, 2018. All households who complete the survey by the deadline will be entered into a raffle for four $50 Giant gift cards and twelve $25 Giant gift cards. Surveys submitted after this date will not be eligible for the raffle. We will draw names and notify the raffle winners in early July. In order to ensure delivery of the gift cards, please provide your property and unit number (questions 1 and 2) when completing the survey. Only one survey will be accepted per household.
