1. Michigan Technological University is asking you to provide information so we can learn more about how you feel about science.
2. If you agree to be part of this survey, you will answer several questions about your feelings regarding science in general and your plans for the future. This survey may take about 20 minutes for you to complete.
3. There are no known risks for participating in the survey.
4. There is no direct benefit from participating in the survey, however your answers will help provide information that could be useful to students taking the course in the future.
5. If you don’t want to participate in this survey, you don’t have to participate. Remember, being in this study is up to you and no one will be upset if you don’t want to participate or even if you change your mind later and want to stop. Participating in the survey is not a required part of any assignment in your science program.
