Letter of Informed Consent
Purpose/Objectives of the Study
My name is Dr. Iris Lesser and I am a researcher at the University of the Fraser Valley working with Dr. Lisa Belanger founder of Knights Cabin Cancer retreats. We are conducting an online survey to learn about your physical activity behavior, environmental preferences and health.
Procedures involved in the Research
Purpose/Objectives of the Study
My name is Dr. Iris Lesser and I am a researcher at the University of the Fraser Valley working with Dr. Lisa Belanger founder of Knights Cabin Cancer retreats. We are conducting an online survey to learn about your physical activity behavior, environmental preferences and health.
Procedures involved in the Research
If you choose to consent to completing this online questionnaire you will be asked about demographic variables, your cancer history, physical activity and environment, nature and questions related to health related quality of life, fatigue, and happiness. It will take approximately 30 to 45 minutes to complete the online questionnaire.
Potential Benefits
We do not believe that there will be specific benefits to you by completing this questionnaire but that it will help to inform community physical activity programming for cancer survivors.
Potential Harms, Risks or Discomforts to Participants
You will not experience any physical risks or discomforts, and no psychological risks are expected.
Confidentiality
Data will be kept on a password protected computer and raw data will be destroyed within 7 years of collection. If you choose to provide an email for future research this will be kept in a file separate from your data to maintain anonymity.
Participation
Participation is entirely voluntary and you may stop completing the questionnaire at any time by closing your web browser. Data is anonymous and your data will not be analyzed if not completed.
Study Results
Study results will be used to inform physical activity programming for the cancer community and will be published in an appropriate academic journal where it will be read by a larger audience.
