Veterinary Prescription Practices Survey

Colorado State University (CSU) and the Colorado Integrated Food Safety Center of Excellence (CO CoE) are conducting a survey that will evaluate production animal veterinarian prescription practices.

This survey should take approximately 15 minutes and will be composed of scenario based questions (6-7 questions) and demographic questions (4 questions). Responses will be kept confidential and participation in the survey is voluntary.

At the conclusion of the survey, there will be an opportunity to learn about keeping a prescription diary. Compensation for keeping a prescription diary will come in the form of an electronic tablet device. Additionally, there will be a chance to enter to win one of several Amazon gift cards for completing the survey.
