Consent to Participate

Hello. My name is Daniel Regan. I’m a research psychologist. I’m interested in how people with chronic conditions value their medications. The title of my research is “Surveying the views of patients about their health products”.

Why am I doing this research?
What patients think about their Diabetes medication is important for the development of health products for Diabetes, and other healthcare decisions. I want to find a better way to capture this than currently exists, and perhaps improve how key decisions are made in the future.

What happens if you choose to take part in this research study?
If you choose to be part of the study, I will ask you to complete a brief survey (about 7-10 minutes). The survey asks some broad questions about your experiences of using Glucophage.

Benefits to you for taking part
If you choose to take part, this study will give you the opportunity, maybe for the first time, to express your opinions about Glucophage.

Risks to you for taking part
I won’t be asking you any overly sensitive or personal questions so I don’t believe that your participation in this survey will bring up distressing thoughts or memories. However, any research participation can cause discomfort. For example, it may the first time you’ve talked about these issues.

I will provide my contact details so that you can contact me with any issues that come up for you. I will also provide contact details of patient support groups, in case they may be of benefit to you.

Your data, and privacy issues
Your raw data will only be seen by myself and another researcher working with me. I won’t be requesting your name or other identifying details other than your age and gender. Your data will simply be pooled with your peers, so that I can identify general findings. All the information you give in the survey will remain confidential and not identifiable in any of the reports written about the research.

Withdraw from the study
Your participation is entirely voluntary, and if you agree to take part in the study, you can choose to withdraw at any stage, without any complications, until your data are no longer identifiable. If you agree to participate, you can simply click to enter the survey, and we will take that as a positive indication that you are happy to continue, and that you have read and understood this information sheet.

How will you find out what happens with this project?
Once the study is over, I will be writing up a report on the key themes we identified. If you require a copy of the written report, you can contact me in 2018.

Contact details for further information
If you require any more information about the study, please contact me at medicationstudy@hotmail.com. If you prefer, I am happy to call you on a phone number that is most convenient to you.
