Reporting High GER's

Host Home Providers must immediately report the following to the Ariel Case Manager, if occurring during normal business hours, or to the Ariel on-call, if after hours. Once reported, a high GER must be written.

1. Any observation or allegation of MANE (Mistreatment, Abuse, Neglect, Exploitation) of an individual.
2. Any incident that involves a sexual issue in public, in front of other individuals, or towards another individual.
3. Any unusual behavior towards, or in the interest of children.
4. Any 911 calls.
5. All hospital visits or admittance.
6. All medication errors.
7. Any incident that involves law enforcement.
8. Any assault or extremely disruptive behavior to another person or property.
9. Any use of a weapon.
10. Any highly disruptive behavior in public.
11. If there is any question if you should report something, then report it.
