By submitting this form as the parent, guardian, or adult relative of the minor, you confirm that the minor has completed all required modules for their age group. Once submitted, the form will be reviewed, and the $10 reward will be deposited into the minor’s FCFFCU savings account by May 15, 2026. Limit of one reward per FCFFCU minor member.
