Introduction and Background

ElderSource is the state designated Area Agency on Aging and Aging and Disability Resource Center for seven counties in Northeast Florida.  As the Area Agency on Aging we are charged with identifying the needs of older adults and their caregivers, developing plans that will help address those needs, contracting with local provider agencies to deliver services and providing the stewardship of the funds.

As we look to the future, we want to get feedback from people across the community.  We appreciate you taking a few minutes to complete the survey.
