Unifying Community Around Self-Determination

Thank you for participating in this survey to share your thoughts about the Self-Determination Program (SDP). SDP gives individuals and their families more freedom, control, and responsibility in choosing services and supports to meet the goals in their Individual Program Plan (IPP).

We used Eastern Los Angeles Regional Center Local Volunteer Advisory Committee Meeting Minutes, program orientation materials, and answers to a state survey from 2023 to create the survey. We’ll use what we learn to improve SDP services and supports in Eastern Los Angeles.
