RE: Strategic Planning Survey -- Respond by March 29, 2010
As you know, the Christian Home & Rehabilitation Center has decided to engage in a strategic planning process to chart the future course of the Center.
Because we want the process to be as inclusive as possible, as a first step we need the benefit of your thinking on a few important questions. Complete and submit this survey by March 29, 2010. We estimate that it will take approximately 30-45 minutes to complete this survey.
Everyone involved with the Center has a different set of skills, expertise and familiarity with the operation of the organization and the needs and trends within the continuum of care for the elderly. Please answer the questions based on your knowledge and familiarity with the Center, the continuum of care for the elderly and the people we serve. All responses will be helpful as we undertake this important process. The survey has six parts. They are described below:
Part 1: First, share your current understanding of the Christian Home & Rehabilitation Center mission.
Part 2: Second, think about internal strengths and weaknesses of the Christian Home & Rehabilitation Center.
Part 3: Third, reflect on recent and future external changes and trends likely to have a significant impact on the Christian Home & Rehabilitation Center, the field of skilled nursing care and the people we serve over next 5 years.
Part 4: Fourth, think about how the Christian Home & Rehabilitation Center is viewed by our key constituent groups as well as the changing needs and service expectations they have of the organization. "Constituent groups" refer to those people who use, support and/or benefit from the Christian Home & Rehabilitation Center.
Part 5: Fifth, think about organizations with which we may compete or collaborate in the future.
Part 6: Finally, identify the most critical strategic issues and challenges facing the Christian Home & Rehabilitation Center over the next several years.
Survey responses will be compiled into a summary for use by the Strategic Planning Committee during the planning process. To assure confidentiality, returned surveys cannot be correlated with email addresses so comments cannot not be attributed to individuals.
