You are invited to take part in a survey about returning to sports and recreation after acquiring a physical disability. The purpose of this survey is to better understand how factors such as time since diagnosis, personal supports, and barriers influence someone’s readiness to participate in adaptive sports and recreation. The information gathered will help organizations and professionals better connect people with adaptive recreation opportunities and reduce barriers to participation.
Participation is voluntary. Providing personally identifying information is optional, and any identifying information you choose to share will be kept private. We may share quotes from open-ended responses with our community, but these will not include identifying information.
