National School Lunch / Food Service Survey

Please help us determine our school food service supply needs by filling out this survey. We will be serving Breakfast, Lunch and Snack daily. If you are keeping your child home, you may pick up your child's food each day. We would have pick up each day and you would receive a breakfast, lunch and snack in the bag for each day's pick up.
