Your experience - We would love your feedback

The staff at the RHW are gathering information about the service we provide in order to find out if we are meeting the expectations of the women who entrust their care to us, and so that we can continue to improve our services and meet the needs of our community. We would value your opinion but you are not obliged to complete the survey. If you would like to contribute to our evaluation we have developed a short, anonymous survey for you to complete online at www.seslhd.health.nsw.gov.au/rhw.  Please be assured that your email address will not be identified so that any comments you make cannot be linked back to you.
Whether your care was provided by the hospital midwives, doctors, your local GP, by one midwife or a small group of midwives (Midwifery Group Practice), all information you provide about you, your care, your experiences and your baby, is important to us.
If you have any complaints or concerns about any aspect of your care that you would like addressed personally please feel free to contact the complaints officer on 9382 6822 or via: SESLHD-RHWfeedback.health.nsw.gov.au. Any complaint you make will be treated in confidence and investigated fully and you will be informed of the outcome.
This survey should take about 10-15minutes to complete.
If you, or a family member or friend would like the opportunity to become engaged with the RHW Community Advisory Committee please visit  http://www.seslhd.health.nsw.gov.au/rhw/consumer.asp or call 9382 6846 for further information.
