Pre-screener to be considered

SIS Research is conducting Video Web Interviews with Pemphigus Vulgaris patients in the USA.

To be considered for this study please take the survey below. If you are selected, you will be asked to participate in two tasks. The first task is a 30-60 minutes assignment that you can complete at your suit. Those who complete the first task will get a $100 USD compensation and will be eligible for the 75-minutes video interview. There are two options for the interview. If you complete the interview with the video ON, we will compensate you with a $175 USD award, and if you qualify and want to do a only-phone interview, we will compensate you with $150 USD.

The topic is about Pemphigus Vulgaris and its treatments. The interview is confidential and for research purposes only.

We first would like to see if this study is a fit. If you are interested, please answer the pre-screening questions below.

Question Title

* 1. Personal Information

Question Title

* 2. Different people sometimes respond in different ways to the same medicine, and some side effects may not be discovered until many people have used a medicine over a period.  For this reason, we are now required to pass on to our client, who is a manufacturer of medicines, details of any side effects related to their own products that are mentioned during market research. Although what you say will, of course, be treated in confidence, should you mention during the discussion a side effect when you, or someone you know, became ill after taking one of our client’s medicines, we will need to report this, so that they can learn more about the safety of their medicines.
Please note that: Any information you provide will remain anonymous unless it must be reported to the sponsor. Your participation is entirely voluntary, and you have the right to refuse to answer a question or completely withdraw from the research at any time. Your responses will be used for market research purposes only. Any information presented to you as part of the interview is confidential and should be assumed to be hypothetical, shared solely to explore your reactions.
Are you willing to participate on that basis?
