The goal of this survey is to solicit agenda items for the December 12, 2024, Collaborative Stakeholder Meeting with Medicaid Managed Care Plans (MMCPs), Health Homes Serving Children (HHSCs), Children's Home and Community Based Service (HCBS) Providers, and Children's Care Management Agencies (CMAs). During the 12/12/24 meeting, the State would like to discuss topics related to HCBS and the HCBS Plan of Care (POC) workflow that are impacting HHSCs, MMCPs, HCBS Providers, and CMAs. This is an opportunity for all involved entities to hear the same information and collaboratively work to solve any workflow barriers that may exist. Meeting invites will be sent separately from this survey. We are requesting that agencies collaborate with their staff to complete one singular survey for their organization.
