Youth Wellness Ambassadors Community Service Project - Survey - 2024

This survey is designed by our 2024 Youth Wellness Ambassadors - Fremont high school students who have been selected to participate in our annual community service project to raise awareness and reduce stigma around mental health, and substance use disorder. This year the students are studying how Alcohol, Drugs and Opioids have Impacted Youth and our Community. Please help our Youth Ambassadors by taking this brief (5 minutes or less) anonymous survey. The survey results and other information the students have gathered will be shared in an educational presentation developed by our Youth Ambassadors.
