Community Survey: Respiratory Infections in Patients with COPD

You are invited to take part in a survey about your experience with COPD and respiratory viral infections. You will be asked questions about symptoms you may feel when you have a rhinovirus/enterovirus (RV/EV) infection, also known as a common cold. You will also answer questions about participating in a potential clinical research study. This survey is sponsored and funded by Altesa BioSciences, Inc., a biotechnology company, in collaboration with the COPDF Foundation.

The estimated time to complete the survey is less than 10 minutes. Respondents must confirm that they are 18 years or older, have been diagnosed with COPD and reside in the United States to be eligible to complete this survey.

WHY IS THIS SURVEY BEING DONE?
This survey is intended to collect data which will help design a clinical study to test a new potential treatment for patients with COPD who develop a common cold. The anonymous data will be collected and analyzed by the COPD Foundation and will only be used for internal clinical study design purposes by Altesa BioSciences, Inc.

WHAT ARE THE POSSIBLE RISKS AND INCONVENIENCES?
Your responses are anonymous and are not linked to your COPD360social or Facebook/Instagram accounts. No personal information that could be used to identify you will be collected. Your responses will be identified only by a random ID number assigned when you enter the survey. The information gathered in this survey will only be shared with Altesa BioSciences, Inc. in a summary format. Although we will do our best to protect the information you provide, there is still a very small risk of loss of privacy. Please do not enter any personal information that could be used to identify you (e.g., name, email, phone number) when completing the survey.
