Healthy Ashtabula County - Community Health Assessment Feedback Survey

The Health Departments of Ashtabula County, along with community partners, are currently completing a community health assessment to understand the health issues facing residents. We have conducted surveys, community conversations, and analyzed data which will be used to select priorities of focus to inform a community health improvement plan. Before completing the survey, please take a few moments to review the findings of the draft here.

We anticipate the survey will take no more than 5-10 minutes to complete. All responses will remain confidential and will only be reported as a group summary.

The final assessment and priorities will be released in mid-July. Stay-tuned to the Facebook or website pages of the Health Departments of Ashtabula County for the final version.
