Welcome to the IMC Provider Readiness Assessment Tool
This survey has been developed based on prior IMC implementation experience and is meant to collect information to assess where you are in terms of readiness to work with MCOs. In an effort to improve provider experience and avoid duplication, one survey is being sent out on behalf of all the MCOs.
Please answer the questions to the best of your ability and consult with others in your organization to ensure responses are as accurate and comprehensive as possible.
Feel free to expand in the additional comments section for each question if your answer cannot be captured with a yes or no response or if there is additional detail you would like to share/expand on.
Please note that we are aware there may be some overlap with these questions and the Qualis assessments. We do not have the completed Qualis assessments for each provider at this time and do not want to delay obtaining this information.
