Health Management Associates (HMA) is working with the Colorado Department of Human Services (CDHS), Office of Behavioral Health (OBH) to conduct a statewide needs assessment and prepare a five-year strategic plan. The goals of the needs assessment are to learn about mental health and substance use needs across the state, identify gaps in services or the continuum of care, and identify populations that experience behavioral health disparities. The assessment will take into consideration the perspective of state and local governments, payers and accountable entities, treatment providers, organizations working with specific high-risk populations (e.g., homeless), advocacy organizations, and individuals with mental health and substance use conditions.

In this survey, we invite you to share your agency’s thoughts and experiences as they relate to behavioral health to inform OBH’s understanding of the following:
  • What are the mental health and/or substance use needs in counties, and how are they different among populations experiencing health disparities?
  • What are the challenges with the existing behavioral health system in counties?
  • What are the barriers to creating meaningful change in the behavioral health care system?
  • What resources are available to support county efforts in behavioral health?
  • What resources are needed to support county efforts in behavioral health?
  • What is the role of the State in supporting county behavioral health efforts?
We are interested in the perspectives of county agency leadership and relevant staff who can provide information about behavioral health needs in the county or counties served by your agency. Please respond with the experience of your agency rather than personal experience outside of your role in county government. If possible, we encourage completing the survey with one or more other agency staff to ensure a response that best reflects your agency’s perspective.

Information from the survey will be analyzed and reported in aggregate as part of the final needs assessment report to OBH. The needs assessment will inform OBH’s five-year strategic plan.

All answers are anonymous. Individual survey responses will not be released, nor will they be shared with the State or anyone else. We will not be able to identify the people who took the survey.

The survey will take approximately 15 minutes to complete.
