Tri-County School Survey on E-Cigarette/Vape Disposal

Public Health departments in Clackamas, Multnomah and Washington counties are gathering information from local middle and high schools about how they store and dispose of confiscated e-cigarette/vape devices. This data will guide the development of strategies to help schools safely dispose of vape waste. This effort includes raising awareness in our communities about the health and environmental harms caused by vapes.

This survey should take less than 10 minutes to complete. Questions marked with an asterisk are required. If someone else on your staff is more familiar with this topic, please feel free to forward the survey for them to complete.

No individual school data from this survey will be shared. This ensures your responses remain confidential while providing a comprehensive understanding of current practices.

Thank you for your participation and for your commitment to creating a healthier future for our students.

If you have any issues with the functionality of this survey, please contact Marc Czornij, Clackamas County Tobacco Prevention Coordinator, at mczornij@clackamas.us
