Health Professionals NACDD Primary Care Survey

The National Association of Councils on Developmental Disabilities is asking practitioners to help us understand their experience providing primary care services to individuals with intellectual or developmental disabilities (I/DD) or similar functional disabilities. Your response will help us to better understand the challenges faced by practitioners serving patients with I/DD, and to look for opportunities to support practitioners and improve the experience of people with I/DD in accessing primary care services.
