Pfizer Vaccine Survey

Consent is required by a parent or legal guardian for children ages 12-15 before they can receive their Pfizer vaccine, and all vaccine providers have their own unique immunization consent forms. A copy of the Emergency Use Act fact sheet for the Pfizer vaccine is provided for your review.  As a reminder, in South Carolina, those 16 and older do not need parental consent to receive a vaccine from an authorized vaccine provider.

Based on the information received from parents from this survey, the Colleton County School District will arrange a vaccine clinic on one of our school campuses as an option of convenience for parents and students who are interested in receiving the Pfizer Vaccine.
