CHS Vaccine and COVID Testing Survey

As you may have heard, either the COVID vaccine or weekly testing may soon be mandatory for all companies with more than 100 employees and those that serve Medicaid participants. CHS fits both of these categories, so we need to assess how many of our staff have already had the vaccine. Although we respect your opinions on this matter, CHS has absolutely no control over this government policy. We would greatly appreciate your participation in this survey.
