This is a critical time for Western Sydney Local Health District (WSLHD) and we would like to hear from you about your recent experience during COVID-19.

The survey provides an opportunity for feedback and will ask you to reflect on your work experience over the last six months. You are asked to be thoughtful, honest, and candid when completing this survey.

The survey should take around 10 minutes to complete, with responses remaining confidential.
