Door Creek Church - Spiritual Health Assessment

This brief and anonymous questionnaire is designed to help us know more about the people who call Door Creek Church home. There are demographic and spiritual health questions organized around our eight discipleship values, and a few short answer questions. The more clearly we understand where we are as a church family in our faith, the better we can think through meaningful ministries and fitting next steps to help us grow as disciples of Jesus Christ.
