The purpose of this survey is to identify how many students will be using our virtual school option for this upcoming school year. Also, we want parent feedback on what return to school model they would prefer if we return in Phase 4 of the governor's Return to School Roadmap. Phase 4 in the governor's plan has students 6-12 grade wearing masks all day while K-5 students only have to wear masks on the bus or common areas such as the hallway.

Question Title

* 1. Parent Name- Please type your name below

Question Title

* 2. How many students do you have attending school at DKS?

Question Title

* 3. How many children will you have riding the school bus this upcoming school year? Please know, if our region is in Phase 4 when the school year starts, all students will be required by the state to wear masks on the bus.

Question Title

* 4. The following questions ask whether you will send your student(s) to school depending on different models the schools could use.

IN-PERSON MODEL- If school resumes in-person (face to face),  will you be sending your student(s) to school? If so, please check off next to the grade level your student(s) will be in if they will attend school in-person.

Question Title

* 5. If school resumes in-person (face to face), will you be keeping your student(s) home to do school virtually? If so, please check off next to the grade level your student(s) will be in if they will be doing school virtually.

Question Title

* 6. ALTERNATING SCHEDULE MODEL- Some schools are using alternating schedules to reduce the number of students in a classroom or on a bus at the same time. This means students would be split into two groups. One group would attend Monday and Wednesday, while one attends Tuesday and Thursday. Students in the same family would attend the same days. The days students aren't attending school in person, they would receive instruction virtually on the computer.

If we returned to school using an alternating schedule, how many of your students would you want to use this model? Please check off next to the grade level your student(s) will be in if you want them to use this model of alternating days.

Question Title

* 7. If we used an alternating schedule as described above, would you be more likely to send your student to school?

Question Title

* 8. Or if we used an alternating schedule as described above, would you be more likely to keep your student home to do virtual school?

Question Title

* 9. IN-PERSON FOR ELEMENTARY, VIRTUAL FOR MIDDLE AND HIGH SCHOOL- Some schools are having elementary students attend in-person, while middle school and high school students do school virtually. This allows elementary students to be spread out into different classrooms and have more staff work with elementary students so they can work in smaller groups.

If we returned to school using this model of elementary students being in-person and middle and high school being virtual, how many of your students would you want to use this model? Please check off next to the grade level your student(s) will be in if you would like to use this model.

Question Title

* 10. Similar to the previous model, we could consider having elementary students attend in-person, while middle school and high school students attend twice a week in-person and three days virtually
Would you prefer this model?
