Improving Outcomes for Youth at Risk of Suicide

The Venango County Student Assistance Program and The Garrett Lee Smith Youth Suicide Prevention grant team have partnered in their efforts to reduce the risk of youth (10-24) suicide, improve care, and reduce stigma in Venango County. If your child is a Venango County resident between the ages of 10 and 24, your completion of this survey is greatly appreciated.

Everyone has mental health. Mental health is not just a diagnosis from a doctor, but can be positive ways to taking care of yourself. Asking for and receiving help is just one way individuals take care of their mental health, with or without a diagnosis.

Please take a few moments to fill out the survey below to help with improving service, educating the community, and moving towards zero suicide. All submissions are anonymous and there is an option to share an email address if you'd like more information.
