This organizational survey is intended to collect organizational data as it relates to digital inclusion in your organization and community. The survey consists of 20 questions and takes about 15 minutes to complete. Responses will only be published in aggregated form, no individual responses will be published. Personally identifying data, such as your organizations name, will be kept on a secured, password-protected drive, and will only be used internally and to provide you specific feedback on your organization's digital opportunity capacity. Survey data allows us to provide baseline data to measure growth across digital inclusion themes.
Potential risks of this study include mild psychological discomfort with some of the questions. You may skip any question that makes you feel uncomfortable. You may not receive any personal benefits from being in this study. However, others may benefit from the knowledge gained from this study.
You will receive no financial compensation for your participation in the study.
Your participation in this survey is voluntary. If you do not wish to participate in this survey, there will be no negative consequences associated with your decision.
