The Medical Student Section Advocacy Group of the Pennsylvania Medical Society wants to know the policy priorities of medical students across the state. For the following questions, please identify the extent to which you agree with PAMED's position.

Question Title

* 1. Scope of Practice - CRNPs actively lobby lawmakers and regulatory boards to expand their scope of practice. If enacted, Senate Bill 25 (Modernization of the Professional Nursing Law) would allow CRNPs to practice independently after completing 3 years or 3600 hours under a collaborative agreement with a physician. SB 25 passed the Senate 44-6 and was referred to the house professional licensure committee in June 2019.

Question Title

* 2. Extreme Risk Protection Orders (ERPOs) - More than half of individuals who die by suicide use a firearm to end their lives. ERPOs add another less-stigmatizing mechanism to restrict access to firearms during periods of crisis.

Senate Bill 90 and House Bill 1075 have been referred to their respective judiciary committees.

Question Title

* 3. Maternity Leave Equity - Advocating for fair and equitable paid 12-week maternity leave without salary penalty, publicize and support this policy, and advocate for its adoption throughout the Commonwealth of Pennsylvania.

There are no current PA state or federal laws requiring maternity leave pay.

There are no bills currently in the PA House or Senate

PAMED Position - Accepts that pregnancy is a unique medical condition, and that maternity leave should not be treated as sick leave. PAMED supports elimination of punitive salary policies and negative influences on future career opportunities of women physicians who become mothers

Question Title

* 4. Prior Authorization - A process used by health insurers that negatively impacts patient care and increases clerical workload of physicians and their staff members. House Bill 1194 (Prior Authorization) was announced via a house co-sponsorship memorandum in April 2019. The bill, not yet formally introduced, seeks to reform the current prior authorization and step therapy process.

Question Title

* 5. Vaccine Exemptions - Pennsylvania currently allows vaccination exemptions for medical or personal reasons.

Senate Bill 653 (Increasing Immunization Rates in Pennsylvania) eliminates state religious and philosophical exemptions for student vaccinations. SB 653 was referred to the education committee in May 2019.

Question Title

* 6. Vaccines & Physician Practice - Anti-vaccine activists believe physicians and other medical providers are pressuring patients into receiving vaccines. House Bill 286 (Informed Consent Protection Act) requires that doctors do not discriminate against unvaccinated patients, prohibits providers from requiring that patients / parents sign a liability waiver if they delay or decline vaccination, and prohibits insurance companies from penalizing doctors with low vaccination rates or paying bonuses to doctors who vaccinate more children. HB 286 was referred to the health committee in January 2019.

Question Title

* 7. Marijuana - Medical marijuana was legalized in Pennsylvania in April 2016. Medical marijuana became available to PA patients in February 2018. 

The PA governor's office, led by Lt. Gov. John Fetterman, accepted feedback on the legalization of recreational marijuana through May 2019. The feedback is still under review.

PAMED Position - The Society opposes broad-based legalization of cannabis for medical use and does not have a current position on recreational marijuana.

The Society supports adequate and well-controlled studies of medical marijuana and related cannabinoids in patients with conditions for which preclinical, anecdotal, or controlled evidence suggests possible efficacy.

* 8. LGBTQ Healthcare - Members of the LGBTQ community routinely experience discrimination in healthcare settings. 

Senate Bill 56 (Prohibiting Sexual Orientation Change or "Conversion" Therapy for Minors) prohibits mental health professionals from engaging in sexual orientation change efforts with an individual under 18. 

SB 56 was referred to consumer protection and professional licensure committee in January 2019.

Question Title

* 9. Family Detention Centers - The Berks County Family Detention Center is under investigation by the Pennsylvania Auditor General. Governor Wolf's administration believes it does not have jurisdiction to close the center. Calls for emergency removal of the 24+ families in detention continue in spite of this fact.

AMA Position - The AMA opposes family immigration detention, separation of children from their parents ind etention, and plans to expand detention centers. Advocates to expand access and eliminate healthcare disparities for LGBT Pennsylvanians, and for future research efforts that are specifically designed to investigate LGBT health issues.

Question Title

* 10. Student Debt - Medical student debt contributes to the shortage of doctors in Pennsylvania. It also negatively impacts the diversity of the medical workforce and the ability of physicians to pursue important but lower paying careers in primary care and psychiatry. There are currently no bills in Pennsylvania addressing medical student debt.

PAMED Position - The Society lobbies the state to keep money available for medical education and training. The society also supports state and federal level low interest loans and scholarship programs for financially needy students as well as loan deferment programs

Question Title

* 11. Hydraulic Fracturing ("Fracking") - A method of oil and natural gas extraction that involves injecting fluid into subterranean rock formations at high pressure. 

Regulations require fracking operators to complete and submit chemicals used during the fracking process, but if operators consider a chemical or concentration a 'trade secret', they can withhold this information from public disclosure. 

There are concerns that fracking may have negative environmental and health impacts on surrounding communities and residential areas.

There are several bills currently in the Pennsylvania legislature related to fracking.

Question Title

* 12. Women's Reproductive Health - Pennsylvania was 1 of 23 states to adopt detailed abortion-specific informed consent requirements in 2007.

In 2018, a fetal heartbeat bill (HB 2315) was introduced to the PA House of Representatives and did not pass. In the past few years, several states have passed new laws limiting women’s access to reproductive healthcare.

Question Title

* 13. USMLE - The United States Medical Licensing Examination (USMLE) announced this past year that they are considering make the Step 1 exam pass/fail.

Some argue this will reduce burnout and allow students to focus on other aspects of their medical education.

Others argue it will remove a crucial tool used by residency directors to differentiate and select candidates for residency.

AMA Position - ACGME should promote the selection of residents based on a broad variety of evaluative criteria. Residency program directors must not use NBME or USMLE ranked passing scores as a screening criterion for residency selection. AMA with stakeholders will study mechanisms for transitioning to Pass/Fail.
