2026 Silent Auction Donation

Thank you for supporting the AFDO Scholarship Fund with your donation to the Barbara Cassens Silent Auction. Please fill out a form for each item you are donating if they are different. If you are donating multiples of the same item(s), please indicate that in the description.

Once you fill out this form, instructions for shipping or delivering your items to Columbus will be sent to the contact named in this form.
