Awards and Recognition Nomination Form

SPECIAL OLYMPICS COLORADO

This nomination form is for athletes, volunteers, media and/or corporate sponsors who have dedicated their time and service to Special Olympics Colorado. Monthly winners will be chosen by a committee. Winners will be highlighted on the website, in the Quarterly SOCO newsletter and will become candidates for induction into the SOCO Hall of Fame. Please, nominate any individual you feel deserves recognition for their exceptional level of service, leadership and commitment to the SOCO community.
