Microboards Support Initiative

Application Form

A Microboard can help families plan for the future in a formal way. Establishing a Microboard reassures families that their loved ones will be cared for by people who truly understand and support them. It requires careful consideration and a proactive approach and we look forward to supporting you on your journey to incorporation!

Application Requirements: Are We Ready?

Families who participate in this initiative must be ready to incorporate:

- Families will have at least five (5) friends/family/community members who know the person well, are unpaid, and who agree to be part of the Microboard.

- Microboard members should be familiar with the person’s life plan and goals.

- Members will have an understanding of the principles and values of a Microboard. You can check out our YouTube channel for a variety of workshops. We'll cover some of this during our time together too!

- Ideally, families will already have the support of a Facilitator who will be able to collaborate with the Microboards Ontario project team.

- Families agree to be responsible for the incorporation application fees, NUANS corporate name search, and for purchasing General Liability and Directors & Officers insurance (insurance is highly recommended).

13.This project is being evaluated by Queen’s University. Families and Microboards will be invited to take part in evaluations, group sessions, or complete questionnaires. No identifying information will be shared, and the anonymous results of the evaluation will be made available to all participants.(Required.)

15.I acknowledge that this project does not include the costs associated with the incorporation application fee, NUANS corporate name search, or General Liability and Directors & Officers insurance (insurance is highly recommended).
