Ellevate LA Mentorship Program Match Survey

Welcome to the Ellevate LA Mentorship Program! This survey will act as the foundation of your experience across the next six months. The information you provide here will determine the mentee or mentor you are matched with. We recommend you complete this survey with detail, honesty and thoughtfulness to ensure Ellevate LA is able to secure the best possible match for you. The survey will close without exception on May 31 and you will receive your match in June 2026. For this round, orientation sessions will take place in July and pairs will meet from July 2026 until December 2026.

Ideally, Ellevate LA will match pairs who are in the same area often enough to enable in-person meetings.(Required.)

This will help Ellevate LA determine the importance of a match in the same region as you.(Required.)

This will help Ellevate LA match you to someone with similar career experience or aspirations. (Required.)

Depending on interest, Ellevate LA is considering coordinating socials across different regions to keep energy high throughout the six-month program.(Required.)
