Club Event/ Activity Reporting Form

Instructions:

Thank you so much for being a UUSA club in our network! This form serves to learn a little bit more about what your club has been up to this month.

Filling out this form automatically enters you to win additional UUSA merchandise from us! We will be picking a new set of winners ever so frequently, so make sure to always keep an eye on your email for updates from our team.

When we ask you about the types of events and activities your club has been hosting, here is a little cheat sheet:

-Fundraising: Any type of fundraiser your club has hosted (Example- Candygrams fundraiser).
-Advocacy: Any type of advocacy your club has been involved with (Example- Contacting a legislator/writing a letter).
-General Community Service: Any type of event or activity that has given back to your local community (Example- Book drive).
-Awareness Raising/ Storytelling: Any type of event with the aim to raise awareness around a topic or tell a story (Example- Water Walk).
