LIVE Math Scholarship Nomination Form

OSSM is thrilled to partner with LIVE to give students in Oklahoma access to an engaging and immersive learning experience!

This form is intended for teachers to nominate 6th grade students to be considered for a scholarship to participate in the LIVE math program. Please fill out the nomination form below with the student’s information. Only one nomination is needed for a student to be eligible to apply, and submitting multiple nominations from different educators does not increase a student’s chances of being selected.

If the student is homeschooled, please have a tutor or co-op coordinator fill out the nomination for them.

Parents of nominated students will be sent a scholarship application from LIVE Math. Once the application is completed, they will receive communication if their student has been selected for a scholarship.
