LIVE Math Scholarship Nomination Form

OSSM is thrilled to partner with LIVE to give students in Oklahoma access to an engaging and immersive learning experience! This form is intended for a teacher to nominate as many 6th grade students as they would like to be considered for a scholarship to participate in the LIVE math program. Please fill out the nomination form below with the students' information for them to be considered for a LIVE Math Scholarship. If the student is homeschooled, please have a tutor or co-op coordinator fill out the nomination for them.

Parents of the nominated students will be sent a scholarship application from LIVE Math. Once that is completed they will receive communication if they've been selected for a scholarship.
