CS Summer of Fun Exit Survey for Each Session

Welcome!

Hi, and thank you in advance for completing this survey after your participation in a session during CS Summer of Fun.

This survey should take you about 5 minutes. Please try to finish the survey all in one sitting. You can leave the survey at any time and come back to finish your answers as long as you use the same computer and browser to access the survey.

Please be honest in your answers. The goal isn’t to test you but to evaluate the session and improve it for participants.

The responses we collect from you are only for evaluative purposes and will be used to improve this program. Responses are anonymous. There is no significant risk to you for participating, and the benefit is that we’re hoping to improve this experience for others.

As a thank you for taking this survey, we are offering the opportunity to enter a raffle for a Hannaford gift card valued at $100. Please click on the link on the last page if you would like to enter. Your information will not be shared with anyone outside MMSA, and will not be used for purposes other than selecting a raffle winner.

Each session needs to be evaluated separately. If you would like to evaluate more than one survey, simply complete one survey and then use the original link to complete another.
