Mortgage/Rent Emergency Relief Assistance Pre-Application Questionnaire

This program provides short-term financial support for eligible individuals and families facing urgent housing-related needs, specifically for rent or mortgage payments. It is designed to help one time to prevent eviction or foreclosure during times of crisis, ensuring that individuals and families can remain in their homes.
Our goal is to offer a safety net for those facing temporary financial hardship and help stabilize housing situations in our community. This is made possible through an initial grant provided by the St. Croix Valley Foundation.
Applicants must reside in our service area, provide verification of income, a copy of a rental agreement/lease, and a mortgage or rental statement showing delinquency. Assistance is capped at $500 per application and will be paid directly to the mortgage lender/bank or landlord. Funds are subject to availability.

I am completing this survey to help Wild Rivers Habitat for Humanity better understand the ongoing needs in the community and prioritize future funding efforts.
2.Applicants must reside in our service area. Are you applying for assistance in Ashland, Bayfield, Burnett, Douglas, Polk, Price, Rusk, Taylor, or Washburn Counties in Wisconsin?(Required.)
3.Are you willing to work cooperatively and respectfully with all of our program staff and provide all necessary documentation to process your application?(Required.)
4.Are you able to provide verification of income for all adults in the household to include your most recent tax return, at least 3 consecutive months of pay stubs, Social Security Income, Social Security Disability Income, Child Support, Alimony, VA Benefits, Unemployment, Retirement Income, and any other income not specified above?(Required.)
5.Can you provide a copy of a state issued driver's license or ID card for each person on the lease or title of the home?(Required.)
6.Can you provide a copy of your current lease/rental agreement or mortgage statement and documentation showing proof of your delinquency to pay?(Required.)
7.Do you currently live in income-based housing or have you received or have you applied for housing assistance from other agencies for this same housing need?
8.If you or other adults in the household are not currently employed are you willing to be connected with employment resources such as the Workforce Resource Center?(Required.)
