Host Site Application for OMA Facilitator Training Practicums
Art for the Journey offers long-term care facilities participating in the CMP Grant a special opportunity: to host an OMA Facilitator training practicum in exchange for two additional training certification scholarships (value of $2000). Having four facilitators enables a community to become a thriving OMA site serving multiple care units.
Here is a section from the MOU we ask host sites to sign. Please review to determine if this opportunity is a good fit. If so, answer the questions following.
A. Art for the Journey shall:
Plan all logistics pertaining to the training session.
Communicate with staff members of [ Name of Facility ] 90 days in advance regarding needs for space, room furniture set up, audio-visual equipment needs, and menu for meals.
Publicize the training sessions on OMA's website, social media, e-newsletters, and postcards distributed at professional gatherings.
Allow up to two staff members of [ Name of Facility ] to attend the training at no cost (including CEUs from NCCAP, NCCDP).
*Note, these two staff or facility associates are in addition to trainees who are registered as a part of the LeadingAge Virginia and Art for the Journey Opening Minds through Art CMP funding grant.
Ensure that HIPPA regulations are followed, and provide consent forms for anyone who may agree to allow the use of photography for the purpose of showcasing the training to other prospective participants.
B. [ Name of Facility ] shall:
Collaborate with Art for the Journey OMA staff members to provide the best experience possible for the participants in the training.
Reserve the following spaces for the two-day training:
Large open gathering space to accommodate up to 45 [ Two Dates ], 8 am - 6 pm.
Break out spaces, 3 additional smaller areas each accommodating up to 10 people: [ Date ], 1:00-3:30 pm
Provide a screen or a blank wall in the large gathering space and a microphone (if available) for the duration of the training.
Provide an orientation tour of [ Name of Facility ] to OMA staff in the morning of [ Date ].
Provide meals/refreshments for all attendees and instructors for the following dates:
[ Day 1 ]: Light snacks/drinks at 11:45 am, snack break at 3:00 pm, and art show refreshments (5-6 pm). *
[ Day 2 ]: Breakfast for all trainees (8-9 am), snack break (11-11:15 am), lunch (12-1 pm), light snack break (2:30-2:45 pm). *
Recruit, support, and monitor up to 20 residents (depending on number of trainees) to participate in the OMA program as part of the practicum for the trainees on:

These residents will be partnered one-on-one while participating in the usual OMA program.
Showcase an OMA art show at [ Name of Facility ] (Art for the Journey can provide Art to be hung gallery style or placed on tables).
