Player Evaluation and Feedback Form for Youth Academy.
This feedback is meant to be a tool to help prompt conversation and discussion between player and coach. The score numbers are not significant except to help parent and player measure areas where coach believes the player can improve, and areas where the player is excelling.
Whether good or bad, the feedback is not an endpoint. This does not and should not define the player. This should be the beginning of the next steps in working on reaching a player's best and capacity. Always Becoming....
