Foundational Community Supports (FCS) provides supportive housing and supported employment services to Apple Health (Medicaid) members who face significant barriers, including chronic homelessness, behavioral health conditions, co‑occurring substance use disorders, disabilities, and long‑term care needs.
The purpose of these listening sessions is to gather input from providers on the implementation and impact of the FCS program in Washington State. Feedback will focus on key areas such as access and eligibility, program strengths and gaps, coordination and systems, outcomes, and provider recommendations.
Providers are asked to send up to 2 participants per agency and registration is capped at a maximum of 30 particpants.
