Survey for Hartford Community Members

Dear Community Members,

Thank you for taking the time to share your feedback as we engage in a strategic planning process for the Hartford Community Coalition (hccvt.org). With your help, we will identify ways to improve our services and the impact that HCC is having across our community.

The following survey is anonymous and all questions are optional. This survey will take about 15 minutes to complete. If there is anything else that you think we should be thinking about as we plan for HCC’s future, please reach out to HCC Executive Director, Emily Zanleoni at emilyz@hccvt.org.

Please know that a different survey will be distributed to parents and caregivers in Hartford. If you are a parent or caregiver, we hope you will complete this survey as well.
1.HCC is a collaborative group of community members designed to support and promote the wellness of youth, individuals and families. The Hartford Community Coalition acts to reduce the stigma and impact of alcohol and drug use, food insecurity and mental health in support of the development of a healthy, safe, and resilient Hartford.
